Your First Visit

Patient intake forms can be filled out online ahead of time or when coming into our office. Please list all medications and previous injuries so Dr. Jake can determine the best individual treatment plan for you.

Patient Intake Form

Dr. Jake will take a personal history, perform an examination, and treat the area if necessary. Total treatment time for the first visit is typically 30-45 minutes (subsequent treatments will be much shorter – average appointments are about 20 minutes depending on techniques utilized).

Since your first visit will likely involve moving around, we ask that you bring athletic clothing (shorts and t-shirt if possible).

**We do not currently have the ability to take radiographs (X-rays) in our office. If radiographs are warranted, you will then be referred out to the Pocahontas County Hospital to have the films taken.

We have many different avenues that can be utilized to decrease your pain symptoms. Check out our Techniques page to see the variety of treatments we offer in our office.

We are in the process of networking with different insurance plans. Please contact your insurance company to ensure that all pre-certification and pre-authorization requirements have been met prior to your care in our office.

Most insurance plans require specific deductibles and co-pays to be met prior to paying for any remaining costs. Please remember that co-pays or any deductibles may be due at the time of service.
